Full registration

This type of registration will remove you from your existing registered Practice's list, and transfer your medical record to Cornerstone Surgery

If you are living within the Practice registration area you will then need to collect a 'registration pack' from the surgery or print the following registration forms:

From 2nd October 2017, the GMS1 Registration Form will include ‘Supplementary Questions’ on the back – These questions only need to be answered by patients who are not ‘ordinarily resident’ in the UK, i.e. patients who are not living lawfully in the UK on a properly settled basis
If you are ‘ordinarily resident’ in the UK, you do not need to answer them

To register

In order to register, you will need to bring to the Surgery reception:

  • FULLY completed registration forms
    • GMS1 Registration Form
    • Registration Consent Form
    • Additional Registration Information
  • Proof of identity (e.g. driving licence / passport / bank card / birth certificate)
  • Proof of address (e.g. utility bill / letter from council)

Please make sure when completing the GMS1 form, that you supply your postcode, previous address and GP, or if you are from abroad, you complete the relevant section

We will not be able to offer you an appointment or issue you any medicines for at least 48 hours after registering

If you require medication, please supply either:

  • the right hand side / tear off slip from your prescription
  • a printout of your medication from your previous GP
You may then request a prescription, which will be ready for collection in 2 working days
If you do not supply a list of medication, the Practice will have to wait for your medical record to be transferred from your previous GP, which may take several weeks

We would advise that you have a sufficient supply of medication from your previous GP before registering with the Surgery

New patient medical

We invite you to make an appointment for a New Patient Medical, once you have registered with Cornerstone Surgery. Please ask the receptionist when you hand your registration forms in

Summary Care Record
