Your health records

Lawful basis for recording and using your data

The Practice records and uses your information under Article 6(1)(e) of the General Data Protection Regulations, as “processing is necessary for the performance of a task carried out in the public interest or in the exercise of official authority vested in the controller”

Data regarding someone's health is classed as a special category of personal data, and its processing is restricted. The Practice, however, processes this information Under Article 9(2)(h) for the "provision of health or social care or treatment or the management of health or social care systems and services"

What information we record

We keep personal information, such as:

  • Your name, address, contact details, date of birth, NHS number, ethnicity and next of kin

We keep clinical information, such as:

  • when you have attended for appointments
  • details and records about your treatment and care e.g. conditions, problems and medication
  • test results e.g. x-rays and blood tests
  • details regarding care you have received outside the Practice e.g. hospitals and Out Of Hours service, Walk-In-Centre

We also keep information which is relevant to your health & care, such as:

  • safeguarding requirements - Child Protection, Looked After Child
  • legal requirements - court orders, Mental Capacity
  • benefits, Universal credit

How do we obtain information

We keep information which comes from you or your authorised representative (e.g. carer, family members), such as:

  • consultations with clinicians
  • discussions with administrative staff
  • messages sent to the Practice using Online Patient Services (e.g. Patient Access) are recorded directly into your medical record
  • the content of emails, where appropriate, are added to your medical record
  • the Practice does not record telephone calls, but will add details of telephone conversations to your medical record

We keep information from other organisations & services who are involved in your care, such as:

  • St. Helens Rota (Out Of Hours)
  • hospitals
  • clinics
  • community nurses

We also keep information recevied from organisations such as:

  • Local council
  • Department for Work and Pensions (DWP)
  • DVLA

Information from other organisations is usually received in the form of a document, but the Practice will add relevant information (e.g. a diagnosis or treatment) to your clinical record

How this information is used

The Practice uses the information in your record to:

  • provide effective care and treatment
  • communicate with you as required e.g. inviting you for appointments, reviews, vaccinations or tests
  • refer on to other care providers as and when required
  • co-ordinate your care and treatment with any other agencies involved
  • conduct searches and audits
  • monitor & review the performance of the Practice

When necessary, the Practice shares information about you with NHS services (local and national) who use this information to:

  • update the national database of NHS patient details such as name, address, date of birth and NHS Number
  • manage call & recall for national programmes:
    • cervical smear screening
    • diabetic retinopathy screening
    • childhood vaccinations
  • maintain shared care records:
    • Summary Care Record (SCR)
    • GP Out Of Hours
  • calculate payment for services provided by the Practice:
    • Quality & Outcomes Framework (QOF)
    • Enhanced Services
  • conduct statistical analysis
    • National Diabetes Audit
  • help plan future provision within the NHS
