Clinical records

The Clinical Record contains details of the care & treatment patients have received, as well a documents received by the Practice e.g. hospital letters

Patient data recorded in the clinical software by the Practice, is stored by EMIS, and documents received by the Practice (e.g. letters from hospitals / clinics) are stored by Docman (PCTI Solutions); this is known as 'hosting'. Hosted data is stored on NHS accredited data storage systems

EMIS and Docman are NHS approved system suppliers, and do not use the data that they 'host' for the Practice, and cannot access the data without permission from the Practice e.g. if they are providing technical support to the Practice

Patient demographic data

This is non-clinical information about patients, such as name, address, date of birth and NHS Number.

Registration data is shared with Primary Care Support England who provide support and administrative services to primary care. Data is also sent to the Patient Demographic Service which is the national electronic database of NHS patient demographic details

Patient record transfer

When a patient registers with a GP Practice their medical record is sent to the Practice. A patient's original paper GP record, should they have one, is sent to the Practice using the NHS approved courier service. GP2GP enables electronic health records to be transferred directly, securely, and quickly from the patient's previous GP practice to a new GP practice

Registration data is shared with Primary Care Support England who are responsible for transferring medical records between Practices. GP2GP information is transferred using the NHS Spine which is a collection of national applications, services and directories which support the health and social care sector in the exchange of information in national and local IT systems
