How we make decisions (Class 4)

Clinical Decisions

Clinical decisions are made by appropriately trained professionals (i.e. the doctors or the Practice Nurse), and are based upon current clinical guidelines (e.g. National Institute of Clinical Excellence (NICE) standards), using the best information, about a patient and / or their condition, that is available to the Practice

Significant Event Audit

This is a process where "individual cases in which there has been a significant occurrence (Not necessarily involving an undesirable outcome for the patient) are analysed in a systematic and detailed way to ascertain what can be learnt about the overall quality of care and to indicate changes that might lead to future improvements"

Patient Survey

Each year the Practice undertakes 'in-house' patient surveys to ascertain the quality of patients' clinical and non-clinical experience at the Practice. The surveys are a series of questions with a choice of answers which rate the Practice's performance, but also includes a section for comments both positive and negative

The results of the survey are discussed at the Management Team meetings and the Patient Participation Group meetings and are used to agree performance targets

Patient Participation Group

The Patient Participation Group is made up of patients of Cornerstone Surgery who meet with the Practice Manager and other Practice Representatives. One of the functions of this group is to discuss the results of patient surveys, and agree targets and development plans. The performance of the Practice against previously agreed targets is also discussed

External Audit

Evidence to support the Practice's achievement in the Quality And Outcomes Framework (QOF) each year, is submitted to St. Helens Clinical Commissioning Group (CCG) for independent review. The CCG also receives and reviews copies of the minutes from the Patient Participation Group and anonymised copies of a selection of the Practice's Significant Event Audits

The Practice has registered with the Care Quality Commission (CQC), and was inspected by the CQC in March 2015

Management Meetings

These meetings are attended by the 'management team' at the Practice; Dr Consiglio, Dr Sutton & Chris Cunliffe (Practice Manager) and cover a range of both clinical and non-clinical issues including Significant Event Audits, finance, performance and development

Practice Meetings

These are attended by all Practice staff and are an opportunity for the management team to address issues raised in the management meetings. They are also an opportunity for staff to raise any concerns or issues that they might have
